Church Registration Form - To register, fill out this form and send it to the address above with deposit. Please register by March 31, 2013. Any forms arriving later will be subject to a $10.00 per camper late fee. |
Camper Form - Needed for every camper and sponsor by May 1, 2013. |
Medical Form - Needed for every camper and sponsor by May 1, 2013. |
Sponsor Approval Form - Needed on all sponsors on church stationary by May 1, 2013. |
Camper/Sponsor Total - Summary of all campers and sponsors with t-shirt sizes and grade needed by May 1, 2013. |
