Church Registration Form - To register, fill out this form and send it to the address above with deposit. Please register by March 31, 2012. Any forms arriving later will be subject to a $10.00 per camper late fee.

Camper Form - Needed for every camper by May 1, 2012. Sponsor Form - Needed for every sponsor by May 1, 2012.

Medical Form - Needed for every camper and sponsor by May 1, 2012.

Sponsor Approval Form - Needed on all sponsors on church stationary by May 1, 2012.
